Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Our Commitment to Your Privacy

Solas Health, PLLC is legally required to maintain the privacy of your protected health information (PHI), to provide you with this notice describing our legal duties and privacy practices, and to abide by the terms currently in effect. This notice applies to every record of care we create or receive on your behalf, regardless of form.

How We Use and Disclose Your Health Information

We use and disclose your PHI for three core purposes: Treatment — sharing information with other providers involved in your care, including specialists, pharmacies, and laboratories; Payment — billing and collecting payment from you, your insurance carrier, or Medicaid/Medicare; and Healthcare Operations — quality assessment, staff training, and practice management activities necessary to run our clinic.

Beyond these core purposes, we may disclose your information without written authorization only in narrow, legally defined circumstances: when required by law, for public health activities, for health oversight activities, in response to a valid court or administrative order, to avert a serious and imminent threat to health or safety, and for workers' compensation purposes. Any other use or disclosure of your PHI requires your written authorization, which you may revoke in writing at any time.

Special Protection for Substance Use Disorder Treatment

If you receive care through our medication-assisted treatment (MAT) program for opioid or alcohol use disorder, those specific records carry additional, stricter confidentiality protection under federal law (42 CFR Part 2) — a higher standard than HIPAA alone provides. We will not disclose your SUD treatment records without your specific written consent, except in the limited circumstances federal law permits, such as a genuine medical emergency. Any disclosure we are permitted to make will be accompanied by a written prohibition against the recipient re-disclosing that information further.

Your Rights

You have the right to request restrictions on how we use or disclose your PHI; request confidential communications by an alternative method or at an alternative location; inspect and obtain a copy of your records; request an amendment to your PHI; receive an accounting of certain disclosures we have made; obtain a paper copy of this notice at any time, even if you agreed to receive it electronically; and file a complaint if you believe your privacy rights have been violated, without fear of retaliation.

Our Duties

We are required by law to safeguard your PHI and to follow the terms of this notice while it remains in effect. We reserve the right to revise this notice as needed, and any revised notice will apply to all PHI we maintain, including records created before the change.

How to File a Complaint

Should you feel that your privacy rights were compromised, you can submit a grievance to our office or the U.S. Department of Health and Human Services’ Office for Civil Rights. Submitting such a report will not impact your treatment.
